It arrives unforeseen: an opening, a shift in how we see and feel. Sunbreak Psychiatry makes room for those openings: unhurried visits, a doctor who knows your story, and direct access to expert psychiatric and brain health care.

The neurobiology and your biography of suffering are not separate problems, so we don't treat them separately. Care here is multi-modal by conviction and personalized to where, and who, you actually are.

Comprehensive Evaluation

Every course of care begins with a thorough, careful evaluation where we cover your life's narrative, history, prior treatments, and what you actually want from getting better, so any plan we make together starts from the whole picture.

Accelerated TMS

Accelerated transcranial magnetic stimulation uses focused magnetic pulses to stimulate brain regions involved in mood, anxiety, OCD, and PTSD. It is FDA-cleared for treatment-resistant depression, OCD, and anxiety associated with depression and is an effective off-label treatment for many other conditions. We can complete treatment in as quickly as a single day.

Esketamine (Spravato®)

An FDA-approved nasal spray for treatment-resistant depression, taken in our office with monitoring after each dose and covered by insurance.

Ketamine

An FDA-approved medication, used off-label for depression, PTSD, and as an adjunct to some forms of psychotherapy. Some people notice change within days rather than weeks.

eCOT-AS

External Combined Occipital and Trigeminal Afferent Stimulation is delivered via a prescription headset worn at home that uses gentle electrical pulses along branches of the occipital and trigeminal nerves, modulating the brainstem circuits involved in mood. Neurolief's Proliv™Rx is FDA-cleared as an add-on treatment for adults with major depression who have not improved enough on antidepressants. Daily sessions happen in your own house; we direct the course and review your data between visits.

Non-Invasive VNS

A handheld device held against the side of the neck for a few minutes at a time, stimulating the vagus nerve through the skin; no surgery, no implant. It is FDA-cleared for migraine and cluster headache; its use for mood, anxiety, and autonomic regulation in PTSD is off-label and still being studied.

qEEG Brain Network Analysis

A painless recording of your brain's electrical activity, at rest and during brief cognitive tasks, compared against an FDA-cleared age-matched normative database. It does not diagnose on its own, but it adds a measurable view of brain function to the clinical picture, and repeating it later shows whether treatment is moving anything. We use the Firefly Evoke™ system and we'll explain plainly where the evidence is thin when discussing this emerging technology.

Medication Optimization

Sunbreak is committed to medication minimalism. We use the lowest effective dose, of the least number of medications, for the shortest duration possible. We can also carefully reduce medications via hyperbolic tapering.

Psychotherapy

Sunbreak takes a therapeutic approach integrating psychodynamic and humanistic traditions with insights from modern neuroscience. Psychotherapy aims to create a relational, depth-oriented space for healing, self-exploration, and growth.

Premera and Regence are accepted for psychiatric evaluation, medication management, and interventional care. No treatment works for everyone; we'll share with you what the evidence best supports.

QUESTIONS WITH STRAIGHTFORWARD ANSWERS

Before you ask.

Am I a candidate for TMS, Ketamine, or Spravato®?

Whether any treatment is right for you can only be determined through an evaluation, and we'll tell you if we think something else would serve you better. We always make plans through shared decision-making.

What does "insurance accepted" actually mean?

It means we bill your insurance for that service. What you pay out of pocket depends on your plan's deductible, copay, and coverage rules. We accept Regence and Premera, and we'll help you check your specific coverage before anything begins, so there are no surprises. Most insurance plans still cover prescriptions and labs we order, even if we're out-of-network.

Do I need a membership for interventional care?

No. TMS, Spravato®, and ketamine do not require membership. Membership exists for people who want ongoing, attentive psychiatric care; interventional treatment stands on its own, and we coordinate with your existing providers.

Do you see people for psychotherapy only? 

Membership provides 40 minutes each month for supportive psychotherapy, treatment planning, and medication optimization. We reserve our limited psychotherapy space for our concierge members. If there is availability, we can contact you from our psychotherapy waitlist.

How does the waitlist work?

Leave your name, email, and what you're interested in. As spots open, we write to people, with priority given to urgency and date of inquiry. Submitting a waitlist inquiry costs nothing and commits you to nothing; it just means you'd like to hear from us.

Where is Sunbreak located, and do I have to live on Bainbridge Island to receive care?

We're on Bainbridge Island, a 30-minute ferry ride from downtown Seattle. Many patients come from Seattle and the Kitsap Peninsula. You don't need to live here; you just need to be in Washington for us to treat you. Concierge services limited to Bainbridge Island and Poulsbo. See the space.

How available are you, really?
How quickly will I hear back?

We commit to responding within one business day to messages received during business hours. In practice, most portal messages and missed calls are answered far faster, often within the hour. We'd rather promise you a floor and beat it than promise a ceiling and miss. Of course, feel free to send a portal message after hours; we'll respond during our next dedicated availability block at the latest, and typically much sooner.

Can I be seen the same day?

We can't guarantee it, but we try hard for next-day visits and often manage sooner. If something urgent is happening, message or call the office; we build slack into each clinic day for exactly this reason. Office hours are Monday through Thursday 9a to 4p.

What are the hours for reaching Dr. Tarver's direct cellphone? (Concierge Only)

8a to 5p, Monday through Thursday, by call or text to his direct cell, or by portal message; calls and texts are generally much faster. Appointments are scheduled Monday through Thursday, during normal office hours.

What happens when Dr. Tarver is away?

You'll always have at least a week's notice before planned time away. While away, virtual care is frequently available or a covering physician handles important messages; you're never without a clear path to help. A few weeks a year are scheduled out-of-office time for continuing education, practice development, and vacation, under this policy.

What if I'm in crisis?

We are not an emergency service. If you're in danger or thinking of hurting yourself or someone else, call 911 or 988 first. Then let us know, and we'll be part of what comes next.

Can I get refills or medication changes by text?

Messages are great for questions, important updates, and scheduling. Medication decisions, including controlled-substance refills, happen in visits, where they can be made carefully.

Is this 24/7 concierge medicine?

No, and by design. We'd rather be genuinely present from 8a to 5p than nominally available at 3am. Sustainable availability is what makes our relationship last. Feel free to text after hours; we'll respond during our next dedicated availability block at the latest, and typically much sooner.
